Patients looking to transfer existing prescriptions to SHCC can now shorten the process by accessing our online transfer request form, which sends your information to our pharmacy without the need to call or pay a visit beforehand. By accessing the Prescription Transfer Request Form, patients have the ability to fill out necessary fields (insurance information, medication type, allergies and personal info) for their prescription needs and submit a form directly to SHCC pharmacy. After submitting the form, patients can expect their prescription to be available within a few business days for pick-up at SHCC.
