Private Insurance & Verification
Three easy steps to using your health insurance at the SHCC:
- Complete and submit the Online Insurance Verification Form.
- Concerned about information privacy? You may choose to complete the paper Insurance Verification Form (PDF) instead and fax (352-392-7620) or e-mail (verify@shcc.ufl.edu) the completed form and insurance card.
- Make sure to submit a copy of the front AND back of the insurance card. You may do this via the Online Insurance Verification Form upload option, fax to (352) 392-7620 or e-mail to verify@shcc.ufl.edu.
- Bring the insurance card to all appointments at the SHCC.
Though not required to access our services, students may wish to continue with a health insurance plan already in place with their parents/family members, or they may wish to obtain a policy of their own. For your convenience, the Student Health Care Center has the capability to direct-bill many insurance companies for covered medical charges, currently contracting with more than 60 companies and 130 individual plans.*
- *EXCEPTIONS: The SHCC Pharmacy currently accepts university-sponsored prescription plans and the following commercial prescription plans: Aetna, BlueCross BlueShield of Florida, CVS/Caremark, Medco and Prescription Solutions; other commercial plans may also be accepted — contact us with questions or for more information: (352) 392-1760. Additionally, physical therapy and massage therapy services are currently only billable through university-sponsored insurance plans, though these services are also available through self-pay.
Prior to coming to Gainesville, contact your insurance company to find out what coverage is provided for the student away from home, keeping in mind that some plans do not cover services in geographical areas other than the home city, or that preferred providers may not be located in the Gainesville area.

Use the toll-free number on the insurance card to find out policy benefits in detail from the insurance company in advance of care.
