Effective Summer B 2014, all newly admitted or re-admitted students meeting certain criteria will be required to show proof of health insurance; visit studentinsurance.shcc.ufl.edu for all details and requirements mandated by the UF Board of Trustees. 

ATTENTION NEWLY ADMITTED/RE-ADMITTED STUDENTS: Completing the SHCC Insurance Verification Form below DOES NOT submit an official waiver for proof of health insurance. Complete that waiver online at studentinsurance.shcc.ufl.edu/waiver-instructions.

Does the Student Health Care Center (SHCC) accept my insurance? 

The SHCC has the capability to direct-bill many insurance companies for covered medical charges. To find out if the SHCC accepts your insurance, we encourage you to provide us your insurance information ahead of time. Patient Financial Services will contact you if there is an issue with the information provided.

To use your health insurance at the Student Health Care Center (SHCC):

  1. Complete the electronic Online Insurance Verification FormUnless otherwise noted, all sections are required. Please complete this form to the best of your ability. Patient Financial Services will contact you if there is an issue with the information provided.
  2. Upload a copy of the front AND back of the insurance card where indicated.
    • If you prefer, you may fax (352-392-7620) or email (verify@shcc.ufl.edu) this information to the SHCC.
  3. Contact the insurance company in advance of care to learn about policy benefits in detail, how your coverage will work in the Gainesville area and more.
  4. Bring your original insurance card to all appointments at the SHCC.

Should I bring my insurance card with me to the SHCC? Do I need to let my insurance company know that I’m going to be seen at the SHCC? Will my insurance company cover my SHCC visit and/or certain services?
