Immunization Policy:
The University of Florida, in compliance with Florida Statute (1006.69) and Florida Administrative Code (6C-6.001-5) requires that all new students show proof of the following immunizations prior to attending UF:

• 2 Measles (Rubeola) vaccines OR 2 MMRs are acceptable
• 1 Rubella (German Measles) vaccine
• Hepatitis B vaccine series
• 1 Meningococcal Meningitis vaccine
  
• International students and those entering the health professions are required to have a current PPD skin test that confirms they are free of Tuberculosis.

Students must complete the University of Florida Mandatory Immunization Health History Form. Specific details and explanations for how to complete the form successfully are included. Read the directions carefully. Please be aware that students will not be able to register for classes until this form is received and approved by the SHCC immunization staff.

UF's Student Health Care Center offers vaccine counseling and education in addition to administering required and recommended vaccinations in accordance with best medical practices for disease prevention. There is a fee for these services.

Immunization Procedure:

1. Students must complete the University of Florida Mandatory Immunization Health History Form as specifically instructed and return it to the UF Student Health Care Center (SHCC) prior to registering for classes.
2. The immunization form will be reviewed and determined complete or not by immunization staff. If the immunization information is complete the student will be cleared and can register for UF classes when they attend Preview/Orientation.
3. If there is missing or inaccurate information, the student will be notified and their ability to register will be blocked until immunization deficiencies are corrected.
4. Laboratory evidence in the form of blood titers for Measles and Rubella immunity is also accepted in lieu of proof of immunizations.
5. Students can choose to officially decline the Hepatitis B and/or Meningococcal vaccines after first reading specific medical information and then signing a waiver to decline the vaccines.
6. A formal Medical Exemption may be indicated for persons with certain medical conditions and/or contraindications to vaccines. Medical contraindications may be permanent or temporary. Review the CDC's Vaccine Information Statements below for medical contra-indications to the required vaccinations.
7. A Religious Exemption may be indicated for those persons who express specific religious beliefs and practices that preclude from receiving vaccinations.
8. If born before January 1, 1957, an age exemption applies for Measles (Rubeola) and Rubella (German Measles), only.
9. Students will not be officially cleared to register for classes until UF's immunization requirements are met.

Medical Exemptions:

For complete and specific information about the vaccines and medical contraindications to them, carefully review the Centers for Disease Control ‘s Vaccine Information Statements (VIS) listed below for each required and recommended vaccine. A letter from the students physician or the official State of Florida Medical Exemption form must be completed and submitted.

Religious Exemptions:

If the student wishes to decline vaccines based on religious beliefs, the student must read, agree to, and sign a formal letter attesting to their beliefs and acknowledging their risks. An official copy of the letter can be obtained from the SHCC at the time the student attends UF's Preview/Orientation or call 352-392-1161 ext.1-4259.
