Physical Therapy at the UF Student Health Care Center, operated by the UF Health Rehab Center, hosts trained physical therapists who are available to students via referral for short-term care of acute injuries. PLEASE NOTE: Charges apply, and co-pays are due at the time services are rendered.
Referrals, appointments, forms and charges
In order to receive treatment, first schedule an appointment with your SHCC provider/team or outside medical provider to get a referral to Physical Therapy for your condition.
Once you have your referral, call Physical Therapy to schedule an appointment: (352) 294-3065. You will need to bring a completed Information Acknowledgement Form and Medical Intake Form to your appointment.
PLEASE NOTE: Charges apply, and co-pays are due at the time services are rendered. Charges cannot post to your myUFL account for later payment.
- Physical Therapy is now able to bill many insurance companies for services. If you’ve been a self-pay patient but have insurance, ask if yours is now accepted.
