CHARGES FOR SERVICE

Are all services at the Student Health Care Center (SHCC) free when you pay the health fee?

No. When you choose to receive medical services from the SHCC you are legally and financially responsible for any charges related to the care provided at your visit. However, some medical services are provided At-No-Cost to you via Health Fee supplement.

What is the health fee and what does this fee cover?

The health fee is not considered health insurance.

The health fee is a service fee paid as part of your tuition that goes to many campus-wide health initiatives. The health fee DOES NOT cover all possible medical charges at the SHCC. At the SHCC, the health fee supplements patient financial responsibility associated with most SHCC office visits (for example, when a medical provider evaluates a sick patient). What if I’m not currently taking classes or paying the health fee?

Off-semester students, spouses/domestic partners, and students who do not pay the health fee as part of their tuition can still receive services at the SHCC, however, you will no longer have the health fee supplement your financial responsibilities.

UNINSURED: No payment is due at the time of service, however, patients will receive a paper bill in the mail for the final amount due from Florida Clinical Practice Association.

INSURED: Patients will be billed for the visit according to their insurance, including any co-pays, deductible or out-of-pocket costs. Payment is not due at the time of service; however, patients will receive a paper bill in the mail for the final amount due from Florida Clinical Practice Association.

If I pay the health fee, what do I still need to pay for?

Patients are financially responsible for items not covered by the health fee; such as but are not limited to: health insurance, physicals, procedures, X-rays, lab tests, DME (medical equipment), prescriptions, non-prescription medications, vaccinations, massage, and physical therapy.

External service provides not associated with the SHCC are not covered under the health fee. For example: UF Path labs (UF Pathology Laboratories pathlabs.ufl.edu), Quest Diagnostics (www.questdiagnostics.com,) LabCorp (www.labcorp.com.) Patients may receive a paper bill from one or more of these companies.

Patients may incur a no-show charge of $25 if they do not arrive on time, are 15 minutes late or do not cancel.

Will I be required to pay anything when I arrive at the Student Health Care Center?

With the exception of physical therapy, you are not required to pay for services the same day at the SHCC. (Currently, we are unable to accept same-day payments for most medical services.) Pharmacy items may be paid for same-day at the cashier in the SHCC Pharmacy or may be deferred to the patient’s UF account.


BILLING & PAYMENT

Will you bill my insurance company for me?

The SHCC will bill your insurance company for you if you have submitted all related insurance information (see Online Insurance Verification Form) to us by the end of your visit.

We recommend reviewing all insurance benefits and exclusions before your first visit. Contact your insurance company and provide them our tax identification name and number (Florida Clinical Practice Association, 591680273) to learn how your policy will work at our facility.

You are financially responsible for service charges that are not covered by your policy or the health fee.

How will I know if my insurance company has paid my bill?

Your insurance company will send you an EOB (explanation of benefits) via email or paper mail. This will explain how they decide the payment amount and your responsibility. Any patient responsibility will be sent to your bursar account or MyChart.

Will I need to follow up with my insurance company?

Yes. Occasionally, you will need to contact your insurance company to confirm claim status and provide them with requested information.

When communicating with your insurance company, you will need to provide them with information on your service provider or clinic. Have this information handy when you call (facility name: Florida Clinical Practice Association and Tax ID 591680273), date of service, charge amount (s), and claim number if applicable. Always document important information shared during the call, such as: claim status, date of any payments, the representative’s name and reference number.

What if I disagree with how much my insurance company has paid on my bill?

You have the right to appeal your insurance company’s decision. You can follow directions on their website, or you can contact member services at the number on the back of your ID card. Filing an appeal will not guarantee that the insurance company will pay more on your bill, but the claim will be reviewed for reconsideration.

When will I see charges for SHCC services?

Charges not covered by the health fee or by your insurance are sent to your bursar account.

If you are an off-semester student, spouse/domestic partner, or do not pay the health fee as part of your tuition, you will receive a bill from Florida Clinical Practice Association.

INSURANCE


UF INSURANCE REQUIREMENT: Visit UF’s health compliance website for current information and deadlines related to the university insurance requirement.

Do I need insurance to use the SHCC?

Though not required to access services at the SHCC, the University requires all students to carry some form of health insurance coverage.

Does the SHCC accept my insurance?

Though not required to access services at the SHCC, be aware some insurance policy may have limitations. To find out if the SHCC is in your insurance network, we encourage you to provide us your insurance information ahead of time via the Online Insurance Verification Form.

The SHCC Pharmacy also accepts a variety of commercial prescription insurance plans. Contact the SHCC Pharmacy either in person or directly at (352) 392-1760 with any questions regarding prescription coverage.

Should I bring my insurance card with me to the SHCC?

YES. Your insurance ID care holds vital information needed for proper billing and prevent delays in determining your responsibility. You can sent this information using the link below or via MyChart.

How do I enroll in the UF insurance plan?

You will be automatically enrolled in the student health plan if you are taking a minimum of 6 credit hours, and taking in-person classes. If you have your own insurance, you will need to submit a waiver. Please visit Health Compliance Office » College of Medicine » University of Florida

Do I need to let my insurance company know that I’m going to be seen at the SHCC?

YES. The SHCC encourages you to contact member services for your insurance company prior to visiting our facility. They will be able to confirm we are in network and provide guidance on accessing care if out-of-network (prior authorization or a specific agreement for the SHCC providers). Can the SHCC care for my spouse/domestic partner?

Spouses/domestic partners of students may receive care from the SHCC; however, services are not available for children. (All patients must be between the ages of 16 and 64.) For more information, click on Spouse/Domestic Partner Care.
