The CWC will schedule a brief consultation and referral appointment with a counselor. At that appointment, please request a referral to Student Health Psychiatry (SHP). At this time, SHP is located on the first floor of the CWC Building but will be moving this year.

To request an Authorization to Release Health Information form (ROI), please call, email or send a message through MyUFHealth (MyChart). Our office is utilizing DocuSign to safely send and receive protected documents for signature. Reception staff will send the form via DocuSign to your UF email address.
