The SHP case manager will meet with SHP patients after a psychiatric hospitalization. During the Case Management Post Hospitalization appointment, the case manager will conduct an intake with the student to ensure basic needs are met, a safety plan is completed, and additional resources are provided as needed.
Follow-up appointments will be scheduled with all post-hospitalization patients. In many cases the case manager will also work very closely with family members or care takers of all post-hospitalization patients.
