Before you may begin your duties or see patients, you must complete all the steps noted below. This process should take approximately 60-90 minutes.
1. Click here to complete and submit the Trainee Registration Form.

2. Complete necessary online training courses and the Observance Form then return to the SHCC.
3. Complete the Confidentiality Agreement and return to the SHCC.
4. View the Orientation Presentation and submit the Certificate of Completion.
